    This page contains all resources for Section 504 students.  Please read the 504 Parent Information Sheet (1 page) and Notice of Rights of Students and Parents Under Section 504 (2 pages) and feel free to download for a personal copy.  If you would like to pursue a 504 Plan for your child, please contact me at 706-592-3730 ext. 3005 so we may begin the process.  The first part of the process begins with the required two documents found in the links below.  These two documents need to be completed and submitted to the school, to the attention of Theresa J. Williams, Assistant Principal. Once I receive the documents, I will contact you to schedule an Initial Eligibility meeting where the 504 Team will decide if the student is eligible for 504 services.  If found eligible, the team will create the student's 504 Plan at that time.  You will receive a copy of all documentation discussed in the meeting for your records and student's teachers will be informed of the new 504 Plan with accommodations within one to two days.  From this point forward, the school's 504 Team will contact you yearly to conduct an Annual Review of the plan.  The student's 504 Plan is a living document that can be revisited and amended at any time during the school year with parental consent.
