Georgia Immunization Requirements 

Health Action Plans - Parents/Guardians:

PLEASE NOTE:  All Health Action plans are required to be updated at the beginning of each new school year. Parents are to provide the school with any emergency medication listed on the emergency health care action plan (ex. EpiPen, Inhaler, Benadryl etc.)

Place completed Health Action Plans and any necessary emergency medication in Ziploc bag and turn into the clinic on the first day of each new school year. Medication should be in a prescription-labeled box. Any over-the counter medication listed should be in the original container.

REMEMBER:  Both parent and physician signatures are required on the Health Action Plan.

It is recommended that parents print out any needed Health Action Plan forms from the DFA Website or RCBOE website over the summer, and take them to students' physicians for completion for the upcoming school year.
