"I Need Help" Forms

An "I Need Help" form is something that students in grades 3-5 can fill out in order to request help from the counselor anonymously. Students in 4th grade can submit the form to Miss Branson's mailbox outside the counselor's office. For students in 3rd and 5th grade, they can submit the form to Mrs. Hooper's mailbox, also outside the counselor's office. After a student submits an "I Need Help" form, the counselor will call them out of class when they are available to talk.

For students in grades PreK-2, they can discuss their problem with their teacher, and the teacher can reach out to the counselor for assistance. Any student can be referred to the counselor by their teacher, a parent, or themselves. An example of what an "I Need Help" form looks like is pictured below
