ATTENTION PARENTS! Please make sure you take a moment and fill out a new and updated health card for the 2023-2024 school year. The health card alerts me of any health issues to care for your child while in school and whom to notify in the event of a medical emergency.
-
If your child has diabetes, seizures, asthma, or allergies that require the use of a epi-pen, a action plan completed by your child's doctor will be needed along with the emergency medication. The action plan tells us what to do, when to do it, and how to do it in the event of a medical emergency. Your child's safety and well-being is our number one priority!
-
Please also take the time to review the RCBOE medication policy. Medications are not to be dropped off at school without notifying the nurse and proper documentation signed by the physician and the parent. The only medications students may carry on them at this time are inhalers and epi-pens, if a form signed by the physician and the parent is submitted specifying the student has been trained on proper use and is deemed responsible enough to do so.

The medication policy, authorization to give medication form, health card, the policy on when to keep your child home, and action plans for you to review and print when needed is attached below. Please don't hesitate to contact me if you have any questions or concerns! :)
