Drug Testing Information:
Drug testing is required for all students in order to become a TSV Employee. The address for this location is:

Appointments are not required but they are preferred. If you choose not to make an appointment, walk-ins are welcome only until 3:00 pm. To make an appointment please call the number listed below.
The two page document with the Center for Occupational Medicine logo must be filled
out and brought with you.
Please bring a form of identification with you; student ID, license, etc.
If you are under the age of 18, you must have your parent or legal guardian sign the paperwork.
o Please note that if you are under the age of 18 and your results are positive, your parent will be contacted.
If you are under the age 18 and do not have the paperwork filled out ahead of time, a parent or legal guardian must be present to sign upon arrival.
Failure to complete and pass a drug test during this week will result in disqualification from the RPM program.
